Hi,I am K.Sathpriya,3rd BDS student.This is an online elog book to discuss our patients health data after taking his consent .This also reflects my patient centered online learning portfolio 
              
                   CASE HISTORY 
Patient Details 
A 50 years old male,Occupation-Agriculture,Resident of Venkatadripalam presented with 
Chief complaints
Fever,
Decrease in output of urine,
Swelling of legs.
History of present illness 
*Patient was apparently normal 5-6 months back,then he developed swelling of body like facial puffiness,swelling of hands,swelling of legs-insidious in onset,gradually progressive-on and off.
*Swelling of lower limbs-pitting type extending upto knee,relived by lying down,aggrevated on sitting and walking.
*Patient came with decreased urine output since 10 days associated with burning micturition.
*Fever since 10 days-Low grade,intermittent associated with chills and relieved by medication 
History of past illness 
*Diabetes mellitus since 10 years 
*Hypertension since 5 years
*Epilepsy 2 years ago
*Not a known case of TB
*No thyroid 
*No asthma
Drug History 
Metformin-500mg BD 
Amlodipine-10mg OD
Levipil-750mg OD
Family History 
No similar complaints 
Personal History 
Diet-Vegetarian
Allergy towards Brinjal,Potato
Appetite-Loss of appetite
Sleep-Inadequate sleep 
Bowel&Bladder-Abnormal 
General examination 
Patient is conscious and Cooperative 
Pallor-Absent
Icterus-Absent 
Clubbing-Absent
Cyanosis-Absent 
Lymphadenopathy-Absent 
Edeoma-Present 
Q/A
*Is there any possibility of Edema affecting other systems?
*Is there any kidney infection behind decrease in the output of urine?

History of present illness Patient was apparently normal 3 days ago,then she developed fever which is High Grade ,Continuous &associated with chills and cold, She is suffering with body pains,Headache since 3 days, Burning micturition since 3 days, Pain I'm Abdomen since 2 Days ,intermittent ,No aggrevating and relieving factors.
