A 29 years old female,Occupation-Customer service executor,resident of Mumbai presented with 
Chief complains
Diarrhea,Chronic kidney disease 
History of present illness
Patient was asymptomatic 2 months back then there was onset of Asthma 
Severe pain in legs along with low grade fever 
History of past illness
Hypertension 
Non Diabetic
Difficulty in breathing since 2 months
Epilepsy 2 months ago
No Tuberculosis 
Family History 
Patients mother is Hypertensive 
Personal History 
Diet-Mixed diet
Appetite-Normal
Sleep-No adequate sleep
Bowel&Bladder-Normal
General examination 
Conscious, Cooperative.
Patient was built thin & poorly nourished 
Pallor-present
Icterus-Absent
Cyanosis-Absent
Lymphadenopathy-Absent
Edema-Absent
