                 CASE HISTORY 
Patient Details 
A 73 years old male,Occupation-At Home,Resident of Devarakonda presented with
Cheif complaints
Fever for 1 Day - High Grade Fever,
Decrease in urine output since 1 week,
Abdomen Distension since 1 week,
Cough since 1 month-Dry cough,intermittent.
History of present illness 
*Patient was apparently asymptomatic since 1 month then he had Abdomen Distension,which was insidious onset gradually progressive in nature .
*He had loss of appetite since 1 week.
*Patient came with decreased urine output with burning micturition since 1 week.
History of past illness 
*No Diabetes mellitus 
*No Hypertension 
*No Tuberculosis 
*No Epilepsy 
*No CAD 
*No CVA
*No Asthma
Family History 
No similar complaints 
Personal History 
Diet-Vegetarian 
Appetite-loss of appetite 
Sleep-Inadequate sleep 
Bowel&Bladder-Irregular
General examination 
Patient is conscious and Cooperative 
Pallor-Absent 
Icterus-Absent 
Cyanosis-Absent 
Clubbing-Absent 
Lymphadenopathy-Absent 
Edeoma-Present,but cured after treatment.
